Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team as a Home Health Care Specialist. In this role, you will conduct in-home assessments, perform evidence-based practice screenings, and communicate findings to inform member's PCP of potential gaps in care.

• Conduct comprehensive in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and physical exam
• Perform point of care testing and identify diagnoses to inform care management
• Communicate findings to PCP and identify urgent and emergent situations
• Educate members on disease process, medication, and compliance

• Active NP license and national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
• Bilingual - English/Spanish
• Ability to complete physical requirements of the job, including lifting and navigating stairs
• Reliable transportation for home visits

• Additional licensure in other geographic areas
• 1+ years of clinical experience in highest level of education, clinical setting
• Experience in gerontology, cardiology, internal medicine, or endocrinology
